Actions from equipped items not availble in instance

Description
Equipped items can provide new actions to characters that have them equipped.
These actions do not always appear as available options to a player for their action.
This may be the result of improper handling of gearsets (see #418), but it probably should be investigated to make sure.

Have not been able to reproduce this on dev or prod even when creating a custom item.
